This is a surgical technique that removes hair follicles from one part of the body, called the 'donor site', to a bald or balding part of the body known as the 'recipient site'.
The technique is primarily used to treat male pattern baldness. Hair transplantation can also be used to restore eyelashes, eyebrows, beard hair, chest hair, groin hair and to fill in scars caused by accidents or surgery such as face-lifts and previous hair transplants. Hair transplantation differs from skin grafting in that grafts contain almost all of the epidermis and dermis surrounding the hair follicle, and many tiny grafts are transplanted rather than a single strip of skin. The use of both scalp flaps, in which a band of tissue with its original blood supply is shifted to the continue bald area, and free grafts dates back to the 19th century. Let us begin with the basics. The most important two things are 'donor site' and 'recipient site'. Hair is being taken for transplantation from the donor site. Speaking globally, there are two types of treatment for hair transplant are available, FUT (Follicular Unit Transplantation), FUE (Follicular Unit Extraction). Let us go through the details one by one.
In FUT, we perform a surgical procedure and cut a strip full of healthy hair follicle from the back of the head. That's why it is also known as 'strip' method. Depending on the grade of baldness, the strip can be 1.5-2cm wide and upto 22 cm long. With an average of 120 follicles/square cm, we can collect approaximately 3000-5500 healthy hair in FUT method. After that, we cut them into single graphs and sort out the most healthy ones under magnification. This way we can acquire the most number of non-transected follicular units.
Apart from this, we can have more benefits while using FUT. The hair is taken from a dense zone only, so the hairs are generally not affected by androgenic hormones. The chances of longevity of these hairs are much higher than hairs collected from extended zones. Collecting hair follicles took less time in FUT, it generally takes 40 minutes.
FUE is the latest technique for hair transplant. In FUE, we 'extract' hair follicles one by one using a special needle or FUE punch machine. Typically we can extract 2000-3000 follicles from the dense zone. In case of higher grade of baldness, we may need to extract hairs from the extended zone. Due to the punching and extracting procedure we may have a higher number of non-longstanding follicles compared to FUT. The extraction procedure generally takes 2-3 hours, meaning the patients have to lie down in his/her tummy for a longer time.

The Patient could notice new hair in a month after surgery. Though in the first month, the newly transplanted hair keeps falling and growing but you can observe a significant growth in other 5-6 months.

Advances in wound care allow for semi-permeable dressing, which allow seepage of blood and tissue fluid, to be applied and changed at least daily. The vulnerable recipient area must be shielded from the sun, and shampooing is started two days after the surgery. Some surgeons will have the patient shampoo the day after surgery. Shampooing is important to prevent scabs from forming around the hair shaft. Scabs adhere to the hair shaft and increase the risk of losing newly transplanted hair follicles during the first 7 to 10 days post-operation.
During the first ten days, some of the transplanted hairs, inevitably traumatized by their relocation, may fall out. This is referred to as “shock loss”. After two to three months new hair will begin to grow from the moved follicles. The patient’s hair will grow normally, and continue to thicken through the next six to nine months. Any subsequent hair loss is likely to be only from untreated areas. Some patients elect to use medications to retard such loss, while others plan a subsequent transplant procedure to deal with this eventuality.
Hair thinning, known as “shock loss”, is a common side effect that is usually temporary. Bald patches are also common, as fifty to a hundred hairs can be lost each day. Post-operative swelling of face and scalp, hiccups have also been seen in around 5% of transplant patients.
